Work order tips:

- PLEASE, put the name used for this form, ON, or IN the box. It is the sole way I can identify your fork as yours. 

- The address you provide here, is where the fork is being sent back to, so work, home, shop,  whatever works best for you, all good, just no PO Boxes, as I use UPS for all returning forks..

- Please look for an email response from me after filling this out, generally within a few hours to a day. If you do not get one, check your spam folder, and if it's not there? Please contact me. Why? Because you will never know your fork is done, if you don't.

- Lastly, boxes shorter than 30" are TOO SHORT. Do not use them please and thank you!  =:)
