Work order tips:

- PLEASE, put the name used for this form, ON, or IN the box. It is the sole way I can identify your fork as yours. 

- The address you use, is where the fork is being sent back to, so work, home, shop,  whatever works best for you, all good.

 

- No PO Boxes, as I use UPS. If you have ANY apartment, suite, floor, etc in your address, please use it, any fees assessed from UPS for address corrections will be billed to you. They're the jerks, not me. =:)

- Please look for an email response from me after filling this out, generally within a few hours to a day. If you do not get one, check your spam folder, and if it's not there? Please contact me. Why? Because you will never know your fork is done, if you don't.

- Lastly, boxes shorter than 30" are TOO SHORT. Do not use them please and thank you!  =:)
